Although some people hope to remain at home for the duration of their illness, many people find that admission to an inpatient Hospice Palliative Care setting can be helpful. These inpatient care settings include palliative care units, residential hospices, or dedicated palliative care beds within local hospitals. Inpatient hospice palliative care offers 24 hour care, specialist pain and symptom management, and psychosocial support for patients and families.
The interactive map can be searched by address, city, or postal code, and results can be limited to a certain geographic distance, specified by the user, and contains links to other important details:
- Admission criteria: provides details on what requirements must be met to be accepted to the hospice/palliative care setting. These requirements may include age, reason for admission, prognosis, diagnosis, and doctor referral.
- Patient and family services: provides details on what programs and services are offered to patients and families, what medical needs are accommodated, and what staff support roles are available at the hospice/palliative care setting.
- Facility information: provides details about the facility, area served, number of hospice/palliative care beds, and types of accommodation available.
